Health Center Director Makes Announcement About Upcoming Move

Dear Community Members,


We are excited to announce that the LCO Health Center will begin its move to a new location on April 7th! During this transition, the Health Center will be closed for two weeks.

To ensure you have the medication and care you need during our closure, we encourage you to talk to your doctor about getting a 60 or 90-day refill before we close. This will help ensure you are well­ stocked while we settle into our new space.


Additionally, Purchased and Referred Care (PRC) eligibility is important for accessing services during the move. Please make sure you are PRC eligible before April 7th by ensuring the following:


•             A current tribal ID is on file.

•             Insurance or a denial for insurance is on file.

•             You have seen a provider within the last 12 months.


Additionally, you must live in the Service Area (LCO Members must live in Sawyer County, other tribal members must live within reservation boundaries)


We will be working closely with Marketplace pharmacy during this time. Please note that PRC eligibility will be required in order to pick up any medications at Marketplace.


We anticipate a soft opening for medical, dental, and pharmacy services on April 16th and 17th, if all goes as planned. We truly appreciate your understanding and patience as we work to transition to our new facility.
